Your health in your hands
Our new patient portal, Wheeler Health Connect, is a secure online website that provides you with convenient access to your personal health information.
We use a state-of-the-art Electronic Health Record (EHR) system for many of our programs, which supports our efforts to provide high-quality and well-coordinated care, while also maintaining the records of our clients in a secure and standardized manner.
You and your family may have many complex needs. Perhaps you have a chronic condition such as asthma, diabetes, or heart disease. You or a loved one may be struggling with emotional issues or addiction. Think of Wheeler as your “home” for care, a place where your medical and behavioral health care is managed and coordinated by a team that works in partnership with you to assess your needs, treat your condition(s), and help you reach your health goals.

Medicaid, Medicare, and commercial health insurance welcome. No patient will be denied health care services due to an individual’s inability to pay for services.
