Compassionate Mobile Care for Hartford Residents

Additional Services in Hartford: Testing and Care for Individuals Living with HIV

Individuals in Hartford living with HIV can connect with care through Wheeler’s Wheeler’s ReConnect Mobile HIV Care program. The program is a community-based initiative designed to identify, engage, and re-engage people living with HIV (PLWH) who have fallen out of care in Hartford, Connecticut. The program features:

Our mobile health center brings HIV testing, STI testing, behavioral health screenings, and infectious disease education directly to high-need Hartford neighborhoods.

Includes links to Wheeler’s Hartford Family Health & Wellness Center (FHWC) for antiretroviral therapy (ART), primary care, behavioral health, substance use treatment, dental services, and social supports.

Assists patients with issues such as transportation, stigma, mental health and substance use, unstable housing) through culturally competent support and strong referral systems.

Wheeler's Family Health & Wellness Centers are funded in part by the Health Resources and Services Administration (HRSA), an operating division of the U.S. Department of Health and Human Services.

Wheeler is a Health Center Program grantee under 42 U.S.C. 254b and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).

Medicaid, Medicare, and commercial health insurance welcome. No patient will be denied health care services due to an individual’s inability to pay for services.
