Our providers and care teams at Wheeler's Family Health & Wellness Centers are amazing, and that also includes #TeamOrange, those staff who work on our two mobile medical units, Winnie and Minnie. In the last few weeks, they've been able to effectively diagnose and treat chronic health conditions like Hepatitis C in multiple new patients through our partnership with the Root Center for Advanced Recovery in New Britain.
In a health care environment that can sometimes measure success by quantity and volume, we wanted to highlight this success...mobile care brought important infectious disease treatment to patients who may not have otherwise been treated, also improving the health of the community.
This is also a testament of the trusting relationship we have with Root as a community partner, who referred the patients to our mobile teams working in their parking lot, which is the true "warm handoff" to supporting care we all strive for every day.
In addition, our partnership with Genoa Healthcare, and specifically one particular staff's dedication to tracking down these same patients for delivery and even coordinating delivery of medications to Root, is also a tremendous asset. Truly integrated care, and living our values of compassion, teamwork, and integrity.
